Improving Transition Outcomes for Youth with Disabilities (ITOP)

A large group of stakeholders, federal, state and community agencies and organizations, parents and youth with disabilities help create a cross-agency, multi-year state plan to help youth transition from school to work. The project includes a statewide map of resources for delivering effective youth services and local pilot projects determining effective ways in which youth with disabilities can access transition services through community partnerships and intermediary organizations. The ITOP website offers many resourses and searchable databases on a project/county basis.
