What Is Your Commitment?

The Change Leader Academy: Leading and Executing Quality Improvement Projects is a four-month collaborative. By enrolling in the collaborative, you are committing to participating in the following activities. The key to success is choosing a project that is important to the agency, this is not “additional work” it teaches you a new way of executing changes in your organization to ensure that that the changes you make are improvements.

Informational Webinar

Participate in a one-hour webinar that will provide you with an overview of what to expect from the CLA. Many of your questions will be answered and pre-work for the sessions will be discussed.

Prerequisites

Complete the following pre-requisites prior to the “in-person” workshop (Links to these documents can be found in the post below.)

Two-Day Training Workshop (In-Person)

By joining the Change Leader Academy, you commit to participating in a two-day “in-person” workshop. There are three sessions to choose from. You will select two people from your organization to participate.* If you have not yet signed up, please have your team leader [REGISTER HERE]. The registration deadline is listed for each session.

*The same two people are required to attend both days of the “in-person” workshop. You will then be able to substitute team members as needed throughout the CLA.

Quality Improvement Project

After the training, with support from your coach, you and your team will work on making changes in your organization using the tools and strategies you learned during the two-day workshop.  In the four months following the workshop, you will refine and complete your project.

Post Workshop Teleconference Calls

Four (4) Post Workshop Teleconference Calls will be scheduled for each session. The calls will be based on the availability of those that are in each session. Dates and times will be selected following the in-person workshop.

Coaching Conference Calls

Participants will have access to individual tele-coaching for four months. You and your coach will schedule these to meet your availability. With support from your coach, you and your team will work on making changes in your organization using the tools and strategies you learned during the two-day workshop. 

Cancellation Policy

Since there is no fee to participate in the Collaborative, there is no financial penalty. However, if you do decide to cancel your participation after registering, PLEASE contact Kim Waterman at kwaterman@cibhs.org.

Substitutions are allowed for post-workshop activities! If one of your team members is not able to participate, please substitute another individual.
